FOR ALL PROGRAMS
Program Change Request Form
Any significant changes to your fellowship program (i.e. Program Chair change, addition or deletion of fellow positions, significant curricular or structural changes) must be reviewed and approved by the AHNS Advanced Training Council – use this form to submit those requests.
Trainee Evaluation Form – To be completed by faculty of the training program
Program directors must evaluate the clinical and technical competence of trainees on a regular periodic basis as stated in ATC program guidelines. The fellow’s evaluation documentation must be provided to the ATC as part of the fellow’s final assessment and satisfactory completion of the fellowship’s training requirements.

Newly accredited programs need to complete this form, which will allow the ATC to provide fellowship applicants with important information and specific details about your program.
